Overview
Learning sessions are monthly or bimonthly sessions for primary care clinic teams that are sub-granted to participate in the Arkansas Department of Health’s ACT program. Sessions focus on facilitated learning for implementation of best practices and achieving systems change through application of the Institute for Healthcare Improvement (IHI), Chronic Care model, and NCQA Patient-Centered Medical Home principles, clinical practice guidelines, performance measurement, EHR adaptation, and clinical risk and population management to effectively improve chronic disease patient outcomes.

Objectives
  1. Describe the importance and implementation of management of hypertension in accordance with evidence-based guidelines, with an emphasis on evaluating elevated blood pressure with no diagnosis of hypertension.
  2. Discuss hypertension management with an emphasis on resistant and secondary hypertension.
  3. Discuss the role of clinical risk-stratification with an emphasis on transitioning patients from high clinical risk for hypertension to low clinical risk.
  4. Increase coordinated care efficiencies and risk stratification for the management of patients with hypertension and hypercholesterolemia.
  5. Discuss implementation plan for identification of high-risk patients using pre-visit planning and EMR identification.
  6. Describe best practices used to improve blood pressure (BP) control, including BP self-monitoring.
  7. Discuss implementation of hypercholesterolemia management.
  8. Identify changes that need to be tested as part of Action Period 4.
  9. Develop clinic team plans for Action Period 4.

AMA Designation Statement

The University of Arkansas for Medical Sciences designates this live activity for a maximum of 5.5 AMA PRA Category 1 CreditsTM. Physicians should claim only the credit commensurate with the extent of their participation in the activity.

AAFP Designation Statement

The AAFP has reviewed Arkansas Clinical Transformation (ACT) Learning Collaborative Sessions and deemed it acceptable for up to 14.00 In-Person, Live (could include online) AAFP Prescribed credit. Term of Approval is from 08/13/2020 to 05/06/2021. Physicians should claim only the credit commensurate with the extent of their participation in the activity.

ACPE Designation Statement

These knowledge based activities will provide pharmacists up to 5.5 contact hours or 5.5 CEU. CE credit information, based on verification of live attendance and completion of the program evaluation, will be provided to NABP within 60 days after the activity completion. 

ANCC Designation Statement

The University of Arkansas for Medical Sciences designates this live activity for a maximum of 5.5 ANCC contact hours.  Nursing contact hours will be awarded for successful completion of program components based upon documented attendance and completion of evaluation materials.

*This course is approved for 5.5 of APRN Pharmacotherapeutic hours.

AAPA Designation Statement

The University of Arkansas for Medical Sciences has been authorized by the American Academy of PAs (AAPA) to award AAPA Category 1 CME credit for activities planned in accordance with AAPA CME Criteria. This activity is designated for 5.5 AAPA Category 1 CME credits. PAs should only claim credit commensurate with the extent of their participation.

Long-Term Care and Nursing Home Administrators

The Arkansas Department of Human Services Office of Long Term Care has approved this program for 5.5 continuing education hours.

Certified Health Education Specialist (CHES®)

Sponsored by the University of Arkansas for Medical Sciences College of Medicine, a designated provider of continuing education contact hours (CECH) in health education by the National Commission for Health Education Credentialing, Inc. This program is designed for Certified Health Education Specialists (CHES) to receive up to 5.5 total entry-level Category 1 continuing education contact hours.
