Overview
Learning sessions are monthly or bimonthly sessions for primary care clinic teams that are sub-granted to participate in the Arkansas Department of Health’s ACT program. Sessions focus on facilitated learning for implementation of best practices and achieving systems change through application of the Institute for Healthcare Improvement (IHI), Chronic Care model, and NCQA Patient-Centered Medical Home principles, clinical practice guidelines, performance measurement, EHR adaptation, and clinical risk and population management to effectively improve chronic disease patient outcomes.

Objectives
  1. Develop and present the team Learning Session 3 Storyboard that discusses measurement data, PDSAs tested and implemented, challenges, and best changes during Action Period 2.
  2. 2) Describe changes tested and implemented related to pre-visit planning for high risk patients to include identification, determination of risk level, and interventions planned.
  3. Describe use of a standardized DM planned care at every visit.
  4. Discuss lifestyle modification strategies that need to be integrated into the risk reduction plan for patients with CVD, HTN, and DM.
  5. Discuss use of standardized CVD planned care at every visit, to include blood pressure management, symptom evaluation/management, and complication prevention/management with lifestyle modification.
  6. Discuss use of standardized HTN planned care at every visit, to include blood pressure evaluation, medication evaluation/addition, and complication prevention/management with lifestyle modification.
  7. Discuss the status of the practice’s high risk patient list and the care management strategies used to reduce risk including monitoring, education, self-management goal setting and medical interventions for patients with HTN, CVD, and DM.
  8. Discuss patient disparities including race, ethnicity, gender, and insurance that impact patient care and strategies used to minimize disparities.
  9. Describe strategic changes that support Cancer screening measures, targeting improvement in an additional Cancer Screening Measure for action period 3.
  10. Describe plans for educating other staff at your clinic about ACT.

This activity is for ADH ACT program clinics only, and registration is free.  However, we would still like each attendee to register for accreditation purposes.
