Navigating Transitions of Care is hosted by Arkansas Blue Cross and Blue Shield and will be our 21st annual one-day conference bringing together health care professionals from all disciplines and employment classifications.

Objectives
  1. Interpret the patient experience at a deeper level throughout each transition of care.
  2. Adapt to any deficit between the "planned" care transition vs. the realized outcome.
  3. Identify problem words often used during patient care transitions that may result in regression of the treatment plan.
