Overview

Learning sessions are monthly or bimonthly sessions for primary care clinic teams that are sub-granted to participate in the Arkansas Department of Health’s ACT program. Sessions focus on facilitated learning for implementation of best practices and achieving systems change through application of the Institute for Healthcare Improvement (IHI), Chronic Care model, and NCQA Patient-Centered Medical Home principles, clinical practice guidelines, performance measurement, EHR adaptation, and clinical risk and population management to effectively improve chronic disease patient outcomes.


Objectives
  1. Discuss the development and utilization of population registries including the management of any challenges .
  2. Discuss lifestyle changes and patient prevention of HTN and related BP self-monitoring.
  3. Discuss diabetes prevention and implementation of Pre-DM screening measure.
  4. Examine lifestyle changes that support arthritis and pain management.
  5. Discuss a delivery system design for Transitional of Care (TOC) and strategies used for partnering with Hospitals/ERs.

Registration
Registration is free. However, for tracking purposes we would still like each attendee to register.

AMA PRA Category 1 Credits™ (6.25 hours), AAPA - American Academy of PAs Category 1 CME (5.00 hours), ACPE - Accreditation Council for Pharmacy Education (6.25 hours), ANCC - American Nurses Credentialing Center (6.25 hours), Non-Physician AMA PRA Category 1 Credits™ (6.25 hours)

Accreditation

In support of improving patient care, University of Arkansas for Medical Sciences is jointly accredited by the Accreditation Council for Continuing Medical Education (ACCME), the Accreditation Council for Pharmacy Education (ACPE), and the American Nurses Credentialing Center (ANCC), to provide continuing education for the healthcare team.

 

AMA Designation Statement

The University of Arkansas for Medical Sciences designates this live activity for a maximum of 6.5 AMA PRA Category 1 CreditsTM. Physicians should claim only the credit commensurate with the extent of their participation in the activity.

 

ACPE Designation Statement

These knowledge based activities will provide pharmacists up to 6.5 contact hours or 0.65 CEU.  CE credit information, based on verification of live attendance and completion of the program evaluation, will be provided to NABP within 60 days after the activity completion. 

 

ANCC Designation Statement

The University of Arkansas for Medical Sciences designates this live activity for a maximum of 6.5 ANCC contact hours.  Nursing contact hours will be awarded for successful completion of program components based upon documented attendance and completion of evaluation materials.

 

AAPA Designation Statement

The University of Arkansas for Medical Sciences has been authorized by the American Academy of PAs (AAPA) to award AAPA Category 1 CME credit for activities planned in accordance with AAPA CME Criteria. This activity is designated for 6.5 AAPA Category 1 CME credits. PAs should only claim credit commensurate with the extent of their participation.

 

Registered Dietitians

This program has been approved for 6.50 contact hours of continuing education for dietitians by the Academy of Nutrition and Dietetics.

This Live activity, Arkansas Clinical Transformation (ACT) Learning Collaborative Sessions, with a beginning date of 12/12/2019, has been reviewed and is acceptable for up to  12.75 Prescribed credit(s) by the American Academy of Family Physicians.
