Overview
Learning sessions are monthly or bimonthly sessions for primary care clinic teams that are sub-granted to participate in the Arkansas Department of Health’s ACT program. Sessions focus on facilitated learning for implementation of best practices and achieving systems change through application of the Institute for Healthcare Improvement (IHI), Chronic Care model, and NCQA Patient-Centered Medical Home principles, clinical practice guidelines, performance measurement, EHR adaptation, and clinical risk and population management to effectively improve chronic disease patient outcomes.

Objectives
  1. Develop and present your team’s Achievement Session Storyboard to describe your team’s work during the ACT Program.
  2. Discuss the team’s data and related processes and team roles that have improved your measures.
  3. Evaluate each component of the Chronic Care Model to identify changes your team tested and implemented within each component and changes that still need to be tested.
  4. Describe the most important changes implemented for chronic illness care at your clinic.
  5. Describe the most important changes implemented for chronic illness care at your clinic.
  6. Discuss clinic processes that have improved the most over the collaborative to improve chronic illness care
  7. Describe issues you are targeting for improvement over the next twelve months.
  8. Share what you liked best about the improvement work.

Registration
This activity is for ADH ACT program clinics only, and registration is free. However, we would still like each attendee to register for accreditation purposes.
